A few examples created with BoneStation:

• Standard Report - comes with BoneStation
• Variation to Standard Report - Similar to the standard report but with separate pages for each image.
• Consultative Report - A consultative report with recommendations and impressions.
• Report with VFA - This report shows current and prior scans on first page and includes VFA.

Sample Questionnaires

Here are a few sample bone density questionnaires. Typically FRAX score utilizes the questionnaire. However, the questionnaire can also be used to automate generation of suggested assessment, recommendations, and more.

• FRAX - Basic questionnaire for FRAX
• More Extensive - Similar to the standard report but with separate pages for each image.
• Consultative - More details for a consultative report.

Both reports and questionnaires are highly customizable.
